Cylinder Rent Adjustment Form

By default, cylinder rental is charged to the same funding used for the initial purchase of the gas. This form should be used when funding for the cylinder rental needs to be updated. Requests must be submitted before the first of the month to ensure updated funding is applied for that month’s cylinder rental charge. Requests submitted after the first of the month will be applied to next month’s billing.
