Surplus Request Form

Use this form to offload/dispose of surplus materials through SWAP.

Before submitting this form:

Review our Material Acceptance Guidelines. Departments may be charged a disposal fee for property not accepted by SWAP.
UW capital assets must be approved for disposal by Property Control.

If your list includes capital assets, please include each asset and tag # in the itemized list below.

SWAP pick-up services are restricted to small-to-medium sized loads located on loading docks or in close proximity to the ShopUW+ delivery area. You will be contacted if the pick-up is considered too large to qualify for this free service.

Please note: Current staffing levels may cause delays to our pick-up services. If the pick-up is time-sensitive, please share this information in the Additional Comments section.

Approval is required before material can be delivered to SWAP. Please wait for email confirmation before making arrangements for delivery. Once approval is received, property can be delivered to our location Monday through Friday 8:30 a.m. – 3:30 p.m.

Please upload pictures of the item(s) through this form and include information for the listing in the Additional Comments section (e.g. starting price, desired auction length, dimensions, cosmetic condition, functionality, viewing/pick-up instructions, etc.).

You will be contacted if we have any questions. Once we have all necessary information, SWAP will list the item on the auction site and will send you a link to the listing.

Please be sure to include these photos if you want to sell surplus property from your department's location; it is advised you take pictures of each side of the item and include close-up photos of important components, accessories, or defects.
