MD accounts are for MDS and SWAP-related financial transactions and service requests.

For UW–Madison departments, MD accounts are tied to a funding string and are used by MDS and SWAP to charge/credit for services or sales.

Please be advised that this form will not update your Airgas funding. All funding updates for Airgas cylinder rent should use the Airgas Cylinder Rent Adjustment Form located on the Verona Operations homepage (https://veronaoperations.businessservices.wisc.edu/).

Primary account contact should be the person who can answer funding questions on the account. Department address should be the default shipping address.

Note for state agencies: MD accounts cannot be used to purchase goods from SWAP. Accounts are only to be used for requesting surplus deliveries and recouping auction commissions.

Primary account contact should be the person who will be receiving sales proceeds for property sold on-location. The department address should be the proper address to receive a check from UW–Madison.

SWAP transactions can be processed by providing Workday funding at the time of purchase. An MDS account is no longer necessary.
